New York Department of Sanitation (DSNY) "flusher" truck. I found a matching example that has a visible International Harvester badge, but they might have only supplied the chassis:

The design is reminiscent of these tractors built by Fageol-Twin Coach. These were built on IH chassis like the street flusher in the other thumbnail.

I remember reading about these, very obscure design with less than 100 made I think.

From a preservation perspective, it's kind of unfortunate how few of these designs from obscure makers are still around. But then again, if you were offered a C2 Corvette or a Brown AT-Series which would you pick? :D
