According to Russian wikipedia, it was developed and manufactured by Omsktransmash, and not UZTM.

T-54 based, BTW.

looks like I ain't the only one who though based on T-62  :p

Quote General overview
The SU-122-54 used mechanical components from the T-54A main battle tank but with a reconfigured road wheel layout to better distribute the weight of the forward mounted casemate and its content, further forward than on the MBT. That new road wheel configuration made believed NATO experts for many years this tank destroyer was based on the T-62 chassis.

Only in the T-54/T-55 article subsection:

A very unique and rare vehicle, that has nothing in common with previous Soviet SPG's. Most of them were later converted into the armoured recovery vehicles.

Although relatively simple technology, the manufacture of steel tank wheels involves several processes, and the logistics involved have to be well managed. The main difficulty lies in moving large quantities of extremely weighty steel components about, ensuring that they are all in the right place at the right time. About the time I retired from work, aluminium wheels were taking over in newly designed vehicles. The sprocket rings are another story.

It really looked like one of the soviet SPGs to me, but I failed to find exact match. When @mike962 wrote it was intended to play an ISU-122 I thought "What's the heck? Why would they need to disguise soviet tank as soviet SPG to make it play a wehrmacht vehicle?" But then I saw actors playing german soldiers driving a presumably GAZ 69 with Willys grille I understood that all possible in a soviet movie and decided to wait and it turned out to be a surprise of one of 77 only produced!
