This film was about the Apalachin Mafia Summit Meeting that took place at the home of the Caporegime of the Bufalino Crime Family, Joseph "Joe the Barber" Barbara in Apalachin, Upstate New York on the 14th November 1957.

Over 100 mafia members, consisting of bosses of the crime families, their bodyguards, and their advisers from all over the US, Cuba, and Italy had attended with the aims to discuss the control of their illegal operations, and the division of the Anastasia Crime Family's illegal operations, following the murder of the head of the Anastasia Crime Family, Albert Anastasia who was shot by member of the Profaci Crime Family, Joseph "Crazy Joe" Gallo in the barber shop of the Park Central Hotel in Midtown Manhattan, the previous month on the morning of the 25th October 1957.

The discussion of Anastasia's murder and the earlier murder of Anastasia's associate and the Consigliere of the Anastasia Crime Family, Francesco "Frank Scalice" Scalisi who was shot at a vegetable market in the Bronx on the 17th June 1957 were on the top of the agenda of the meeting as two members who were still loyal to Anastasia and Scalice, Armand "Tommy" Rava and Aniello "Neil Lamb" Dellacroce were about to "declared war" against Vito Genovese and his allies.

Also on the agenda, were the discussion of the reorganization of all the former Mafia Crime Families of Maranzano, Profaci, Mangano, Luciano, and Gagliano into the current Mafia Families of Bonanno, Colombo, Gambino, Genovese, and Lucchese.
