Yes, very far-fetched, but apparentely after test-screenings they thought the audience would find it funnier if Otto survived being run-over by a steamroller and somehow got onto the wing of the jumbo jet so he could once again peek in through the window as Archie and Wanda smooched. Maybe it was very deep cement and the steamroller somehow floating over him, that's closest I can get to a plausible explaination. In the DVD commentary John Cleese said it was OK to have this as it was right at the end of the movie and audiences wouldn't care too much.

They couldn't make the complete road roller for the film, the base is a real road roller, this was Ingersoll Rand DA-50.
