One can't determine the enginesize because of the headlights. These are third type headlights of the Citroën ID/DS, modelyears 1968 - 1975. Since it has the earlier type doorhandles and the second type dashboard, it's modelyear 1968 or '69 (9/67-8/69). It could either be ID19/ID20 or ID21. Most Ambulances were ID19/ID20/DS20. All Break- (Safari-) versions - Ambulance included - were called ID till 1971. Modelyear 1972 and later they were called DS. Although actually there was no difference. 1985 cc Engines were called 19 till 1968. Modelyear 1969 - 1975 they were called 20. After a 10% increase in power.
